Limited Conservatorship Clinic Volunteer Information

Families with a child turning 18 and unable to make their own decisions due to a developmental disability need a court order to continue supporting their child with crucial medical, educational, and other decisions. At our clinic, assist a family in navigating the process of requesting the necessary authority to make these important decisions once the child turns 18.

• Date: April 12, 2025
• Time: 8:30 AM -1:30 PM
• The parking garage is 455 S Main St, Orange, CA 92868. See the attached map of CHOC parking in a red box. CHOC will validate parking.
• After you park, please follow the signs to the CHOC hospital (not the CHOC Clinic). Check-in at reception and join us on the 2nd floor. Security can direct you when you exit the elevator.
• 20 minute video training available.
• Volunteers will get an invitation to the draft system the day before the clinic.
• Please bring a laptop or tablet to complete the forms. If that is not possible, please email rfrance@eldrcenter.org before the clinic.
• Dress code is casual.
